 CONTACT FORM. NYC Department of Education regulations bar the school administration from giving the Stuy PA your contact information. The PA has a lot of information and resources that we wish to share with our parent community, but we are only able to do that if we have your contact information. FUNDING REQUEST FORM. To request funding from the Parents' Association, please complete and submit this form to Room 271 with all supporting documentation. Your request will be considered at the next Approriations Committee meeting.
